My Mount Sinai Chart does not reflect the complete contents of the medical record. To use My Mount Sinai Chart, you must accept these Terms and Conditions by clicking "I Accept" below.
Mount Sinai reserves the right in its sole discretion to revoke your access to My Mount Sinai Chart at any time for any reason. For your privacy, YOUR ACCESS TO My Mount Sinai Chart will terminate automatically after 15 minutes and you will have to re-register to continue using the system. Mount Sinai also reserves the right to discontinue providing My Mount Sinai Chart access altogether.
Response to Electronic Communication –Mount Sinai's staff will make best efforts to provide a timely response to electronic inquiries, however, a patient should allow at least 24 hours, excluding nights and weekends, for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically; instead, call 911 or go to your nearest emergency room.
Web Site Links - My Mount Sinai Chart may offer links to related medical Web sites not managed by Mount Sinai. These Web site link(s) are for a patient's informational purposes only. Mount Sinai does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.
Minor Eligibility - An individual must be age 18 or older to request a My Mount Sinai Chart account. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Third Party Access" section of these Terms and Conditions. Thus, when a minor patient reaches age 12, restrictions will be placed on My Mount Sinai Chart access until he/she reaches age 18.
Third Party Access - Individuals age 18 or older may request third party access to another individual's My Mount Sinai Chart account by completing the Third Party Access portion of the My Mount Sinai Chart Consent Form and submitting it to Mount Sinai. Such access will only be granted to parties who have a legal right to the account holder's medical information or have obtained that person's written authorization. Only one person will be granted third party access to a My Mount Sinai Chart account. Mount Sinai reserves the right to revoke third party access at any time, for any reason.
E-mail Privacy - Users of My Mount Sinai Chart will be notified via e-mail when there is new medical information to be viewed on My Mount Sinai Chart. This means that any person with access to a patient's e-mail account will be able to see this notification. Although no private medical information will be sent, the notification that new medical information is available by accessing My Mount Sinai Chart may be information that a patient would not want others to know. Thus patients should take this into consideration when providing an e-mail address..
Please know that if you send us an e-mail communication, it may be shared with the Mount Sinai staff who assist the physician in providing the patient's medical care. A patient's confidential medical information on My Mount Sinai Chart will be accessible only to appropriate clinical staff.
Security and Confidentiality - We afford the same degree of confidentiality to medical information stored on My Mount Sinai Chart as is given to medical information stored by Mount Sinai in any other medium. Mount Sinai is committed to protecting the confidentiality of your medical information. We limit Mount Sinai employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses My Mount Sinai Chart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. It is your responsibility to select a confidential password, to maintain that password in a secure manner, not to share the password, and to change the password if you believe it may have been compromised in any way.
For other than general information viewing, My Mount Sinai Chart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

INCLUDING BUT NOT LIMITED TO THE ACCURACY, COMPLETENESS, CORRECTNESS, TIMELINESS OR USEFULNESS OF ANY INFORMATION, PROGRAMS, PRODUCTS, SERVICES, OR OTHER MATERIAL PROVIDED THROUGH My Mount Sinai Chart, OR OF RELIABILITY, NON-INFRINGEMENT, MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. These Terms and Conditions, and your use of and access to My Mount Sinai Chart are governed by the laws of the State of New York, without regard to its conflict of laws rules. Jurisdiction and venue for any cause of action arising hereunder shall be New York County, New York.
These Terms and Conditions may be amended by Mount Sinai at any time without notice. Your next use of My Mount Sinai Chart without objection shall be deemed to be accepted of the new Terms and Conditions.
